In collaboration with QueensCare Health & Faith Partnership (QHFP), South Central LAMP is committed to addressing the distinctive healthcare needs of our community through targeted outreach programs. At our site, a registered nurse and community health worker (CHW) are available for walk-in consultations, providing accessible healthcare services. This dedicated team offers low or no-cost immunizations and screenings for both children and adults, coupled with invaluable health education and counseling. Moreover, the nurse plays a crucial role in referring individuals in need of additional medical attention to QueensCare Health Centers or other healthcare resources within the community, ensuring comprehensive and personalized care for all.
